Why do you think you were banned: For having a steam account. Not sure if i was benned on my steam account, the other way round or neither. I didn't think (and maybe can't) use my steam account as it is linked to a banned social club account which gave me issues logging into FiveM from my recollection.
Why should we unban you: Being unaware of the circumstances of the ban i can only ask for forgiveness and i commit to familiarising myself with the new rules. If given more information reguarding the ban i can provide more information.

You will need to appeal both your combat logging ban and ban evasion.
