Firstly, I would like to apologize for the late response/post due to the fact that I was in another country for a month and was unable to respond.

I do remember the event. I started gaining altitude with my helicopter which then got interrupted by "shadow play" tabbing me out of the game. I attempted to tab back in but "shadow play" did not respond and initially caused my entire computer to freeze therefore I closed everything and restarted my computer. When I did come back to the game, I realized what had happened and it was a bit too late. I sincerely apologize since it was an issue from my side and I didn't mean to cause any issues for other players in the game.

Why should we unban you ?
I usually (mainly) go on the server to play a bit of piano and engage in conversations with people. I do not intend to troll around and break any community guidelines. Once again sorry I was unable to respond sooner that was due to the fact that I was out of the country. I will promise that silly mistakes such as this will never happen again since I have fixed the problem. This has been a lesson for me to be more cautious and keep away from accidents such as this.

you got actually banned for logging out after you got injured. the report was up for 2 days. 
Ahy did you not responded on the report. or came to ts after to explain your side?

As I stated I was away for a month and a half because I was in another country. In addition after my return on Sunday, I came on Teamspeak and got told to make a ban appeal. On the report, it says that I committed VDM and then combat logged but as I stated what happened I restarted my pc due for my computer freezing

@Piano Entertainer combat logging right after you get injured, and you blame it on computer freezing? Did your friend computer also freeze at that exact point? Such a coincidence. Out with the truth, or this appeal will not get far.

We were not in combat, I stated, " I accidentally crashed into him due to "Shadowplay"  tabbing me out and then freezing my computer."  As we all know ALT+Z by default opens up "Shadowplay"  while I was ascending the helicopter, I pressed Z because helicopter goes down and ALT because it allows me to look around which then caused "Shadowplay" to open and since it was not responding I had to restart my computer. To add to your statement about me disconnecting, I would not reconnect 5min after if I did want to disconnect to evade a ban.  Unfortunately, If his video on his report still worked I would have been able to describe what happened at each min.  

He even in his report says, "We were flying to the airport to put money away we have just got from a salt run and go do another one after that. We were landing and this helicopter hit into us as we were hovering down."

You've still got to explain the coincidence of both you and your friend disconnecting right after the VDM.

 That was also one big reason why I liked to see the video myself to identify who it was because I usually roll alone. I think it was someone who I attempted to take to the hospital on a stretcher but I'm not really sure. I can confirm I never heard of anyone called "juan-guillen" and you can check on my steam friends but that will not work since arma 3 allows you to alter names. But in all honesty, I don't even know who the guy is

In addition, if he came on Teamspeak or gave me a message to speak to me, I would have resolved the problem and we would have come to a good resolution but the problem is that I didn't receive anything from him and as I returned on Sunday I realized I was banned. I'm genuinely here to have fun, not to troll, the majority of times I sit in the corner play piano. My intentions are not to troll.

In his report, he said he contacted me, but I genuinely did not receive anything.

I sure will. Combat logging is the rule 2.5 within this server and forum:  "Combat logging at any point during RP is exploiting. Some examples of this include: logging out during a firefight, being chased by the police, arrested by the police, while dead awaiting a medic, logging out in order to save gear, rage-quitting, logging out whilst restrained/detained. Punishment is a ban. Staff can make an informed decision depending on if you’ve made an attempt to resolve the issue. We do suggest you attempt to rejoin the server and continue the roleplay (try messaging them with the mobile system first)."

Combat logging is when you leave during an RP situation. 

It can also be:

Logging out of an online game to avoid combat/death. This cowardly tactic often denies the opponent some victory or loot they may have earned for your possible death. This tactic is often used by weaker outfitted or less experienced players.

In this case, It can be seen that I disconnected to avoid punishment but as I stated that was not my intention. Above it also states to attempt to rejoin the server and continue to roleplay which I have done.

to answer your point, I was on Teamspeak during the accident and thought he will contact me. 

Since I was also told my 2 players on teamspeak don't worry about it. I felt like there was no need to tell a staff member. I guess I should have informed a staff member and that was my mistake in this situation.

Do not let it happen again. For safety do inform staff members so you can avoid this in the future if something like it occurs again.
