if a rule doesn't state that it is punishable by ban, do you then receive a warning? 

Because some rules have this stated and some don't? why?

Unless stated otherwise, a breach of the rule would result in a ban.

However some are more serious than others all bans are perm unless appealed (some cant be appealed for).

All violations are punishable by a ban, however a verbal warning (In-game) or written warning (PM) may be issued if the admin deems the violation is correctable and doesn't warrant a ban.
