Inventory gets wiped on re-log stating I was dead at log-out

Basically what the title says. Has happened 3 times now where I re-log from the server and end up getting my inventory wiped with the message attached below.

Can you provide more info. When You left the server where you dead? Sid you have full health? Or what did you do just before you left the server? And please provide Your charakter name.

The 3 times it has happened don't seem to be connected in any way, all just random times without me doing anything specific before logging off. Here is a video of when it happened the 3rd time:
