Denied, as per this post, you must wait 6 months from your initial ban to appeal, and it has to be the other form you fill out:

Seems you did not only combatlog when the cops shot you, but also in this screenshot when you were ziptied and shot by a rebel:

That means you combatlogged twice.. on a cop and on a rebel. Which in turn means I do not believe you are the right player to play on our community.
