Restraint log of player (Action: Ban Issued 28/03/2015)

I understand this is not in format properly but the player would should be banned regardless.

Player's name is 'Shays poo'. He logged out of the game whilst in restraints. I was spectating him at the time but he hasn't re-logged since to my knowledge, so i'm creating this thread before i forget.
