When you bleed out or fully respawn in the city, it respawns you at the Bay Care centre at Paleto, and fully respawning / bleeding out at Paleto causes you to respawn at Pillbox Hospital in the city. In the change logs, it is stated that when respawning, you respawn at the nearest hospital.
